Job Description:
A company is looking for a Case Management Extender.
Key Responsibilities
Assist RN/SW Case Manager in arranging post-discharge needs
Communicate with alternate level of care liaisons to identify
accepting facilities and establish transfer timelines
Arrange referrals for home care services and schedule
post-discharge appointments with providers
Required Qualifications
Associate's Degree or Bachelor's Degree in a medical-related field
or Medical Assistant Certification preferred
Three (3) or more years of work experience required; five (5) or
more years in healthcare preferred
No licensure or certifications required
Working knowledge of medical systems and terminology or completion
of department-approved training within three (3) months of hire
Proficiency with basic computer skills and electronic medical
records (EMR) systems preferred
